About the role
Responsibilities
- Oversee the administration and management of the eStroke Rehab Referral pathway within the RM&R system
- Analyze system data to monitor performance, system flow, and efficiencies
- Identify and facilitate quality improvement initiatives to enhance timely access to stroke rehabilitation
- Provide education and training regarding the referral system to relevant stakeholders
- Troubleshoot system database and process upgrades
- Sustain engagement with acute and rehabilitation partners
- Participate in the Ontario Rehabilitation Coordinators Group to implement best practices across the continuum of care
Requirements
- Regulated Healthcare Professional with a minimum of 4 years of experience
- Essential clinical experience, with a preference for rehabilitation experience
- Knowledge of the Ontario Rehabilitation system
- Certification in common outcome measures (FIM or AlphaFIM required)
- Advanced computer literacy, specifically with MS Office and Excel
- Strong meeting organization and facilitation skills
Preferred Qualifications
- Master's degree in Health Education, Administration, or a related program
- Experience with related project work
- Knowledge of Quality of Care and stroke-specific outcome measures
