eStroke Rehab and Referral Coordinator

Responsibilities

• Oversee the administration and management of the eStroke Rehab Referral pathway within the RM&R system
• Analyze system data to monitor performance, system flow, and efficiencies
• Identify and facilitate quality improvement initiatives to enhance timely access to stroke rehabilitation
• Provide education and training regarding the referral system to relevant stakeholders
• Troubleshoot system database and process upgrades
• Sustain engagement with acute and rehabilitation partners
• Participate in the Ontario Rehabilitation Coordinators Group to implement best practices across the continuum of care

Requirements

• Regulated Healthcare Professional with a minimum of 4 years of experience
• Essential clinical experience, with a preference for rehabilitation experience
• Knowledge of the Ontario Rehabilitation system
• Certification in common outcome measures (FIM or AlphaFIM required)
• Advanced computer literacy, specifically with MS Office and Excel
• Strong meeting organization and facilitation skills

Preferred Qualifications

• Master's degree in Health Education, Administration, or a related program
• Experience with related project work
• Knowledge of Quality of Care and stroke-specific outcome measures
