Enrollment Specialist

Responsibilities

• Identify, screen, and enroll eligible individuals into Health Home and Social Care Network programs
• Manage end-to-end intake process from referral receipt through documentation and handoff
• Conduct telephonic or in-person eligibility screening and verify Medicaid status
• Perform proactive multi-channel outreach via phone, text, and field visits
• Complete enrollment documentation and enter data into Foothold Care Management (FCM) systems
• Coordinate warm handoffs to Health Home Care Managers or Community Health Workers
• Maintain compliance with NYS DOH Health Home standards and HIPAA regulations

Requirements

• High School Diploma/GED with 2+ years of enrollment or outreach experience, OR Associate's degree with 1+ year of experience
• Bilingual in Spanish
• Ability to travel regularly throughout the five boroughs of New York City
• Proficiency with EMR systems, CRM platforms, and Microsoft Office Suite
• Experience with Medicaid, Health Home, or managed care programs preferred
• Strong organizational skills to manage high referral volumes
