About the role

Responsibilities

  • Conduct outreach to customers referred by HRA for the WeCARE program to schedule clinical assessments
  • Maintain customer engagement in program activities such as SSI/SSDI or Vocational Rehabilitation
  • Review medical and health-related documentation with customers to assist in the clinical assessment process
  • Explain program benefits, services, and clinical assessment processes to customers
  • Assess customer needs for accommodations and take appropriate action
  • Assist in scheduling medical, mental health, or substance abuse evaluations
  • Maintain accurate records and scan documents into management information systems
  • Generate real-time reports to assist management with resource assignment
  • Ensure customer files are complete and appropriately signed by health professionals
  • Assist customers with resume development, job applications, and interviewing skills
  • Support care managers with scheduling and unit calendar management

Requirements

  • Bachelor's Degree or higher in social services, psychology, or vocational/occupational rehabilitation
  • Four years of direct customer service experience in a clinical or social service environment
  • Experience in a fast-paced, performance-driven team environment
  • Strong organizational skills and attention to detail
  • Experience with publicly-funded programs (TANF experience preferred)
  • Ability to use various electronic systems for data entry and reporting
  • Experience assessing needs for reasonable accommodations

Preferred Qualifications

  • Training or experience in motivational interviewing and care management techniques
  • Bilingual or multilingual skills
