Endocrinology Nurse Practitioner/Physician Assistant

Responsibilities

• Assist care coordinators across different providers and settings
• Promote better health outcomes through communication between facilities and patients
• Assess patients with endocrine conditions and report through proprietary software
• Cross-train in other areas of care management
• Work under the supervision of an off-site Collaborative Physician
• Gather and analyze patient information
• Perform detailed reviews of the system and pertinent patient labs, studies, and provider notes

Requirements

• Minimum 6 months of experience in long-term care or a skilled nursing facility
• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
• DEA License
• CPR BCLS/BLS certification
• Master's Degree in Nursing from an accredited program
• One year of experience in Endocrinology
• Reliable transportation
• Willingness to travel to different nursing homes within the state

Preferred Qualifications

• Experience documenting in EMR systems such as Sigma, Visual, or PCC
