Embedded Care Coordinator

Responsibilities

• Complete individualized patient care plans and perform care management using the electronic medical record system
• Provide frequent care coordination and support to patients to improve outcomes
• Document all client communications accurately and promptly
• Communicate patient emergent needs to Primary Care Clinicians
• Collaborate with cross-organizational teams to meet patient and family needs
• Manage scheduling and support for caregivers and families

Requirements

• Comfortable with extensive phone communication
• Compassionate and empathetic with strong customer service skills
• Proficient in Microsoft Office, including Outlook
• Knowledge of basic healthcare terms and care principles
• Ability to multitask and remain organized in a fast-paced environment
• Must work full-time, Monday through Friday daytime hours on-site in Wichita, KS
• Able to pass a drug screen and background check
