Responsibilities

• Conduct interviews and assessments to clarify life situations and available resources
• Develop personal resources for coping with illness
• Advise on the design of the living environment
• Advise patients, relatives, and caregivers regarding benefits under SGB V, IX, XI, and XII
• Coordinate outpatient care services, short-term care, inpatient placement, hospice, and other care solutions
• Support decisions regarding advance directives and powers of attorney
• Collaborate interdisciplinarily with all professional groups within and outside the hospital
• Apply for legal guardianship, emergency guardianship, and care/disability degrees
• Advise on disability law and support application processes
• Organize and advise in the area of rehabilitation

Requirements

• (Fach-)Abitur or an equivalent recognized university entrance qualification
• Motivation to take on responsibility and shape the future
