Responsibilities

• Perform standard clinic duties
• Provide advice via telephone and chat
• Handle triage and acute patient flows
• Provide education, support, and advice to patients with diabetes
• Contribute to the development of diabetes care and continuous improvement
• Participate in developing primary care through the implementation of digital-physical processes

Requirements

• Licensed District Nurse or Registered Nurse
• Specialist competence or experience in diabetes care
• Proficiency in Swedish (spoken and written)
• Knowledge of the Cosmic electronic health record system

Preferred Qualifications

• Previous experience specifically within diabetes care
