Discharge & Flow Coordinator

Responsibilities

• Provide administrative and clinical leadership to facilitate the efficient movement of admitted patients through the hospital system.
• Coordinate patient-centered care plans in collaboration with patients, families, and multidisciplinary teams.
• Act as a primary link between the hospital, community agencies, and alternate level of care facilities.
• Manage bed utilization by reviewing incoming applications and monitoring chronic care and nursing home requirements.
• Educate hospital staff on discharge planning resources and act as a clinical resource for acute inter-professional teams.
• Establish and maintain systematic assessments of bed availability to ensure appropriate patient assignment based on clinical need.

Requirements

• Bachelor's degree in a health-related discipline or recognized equivalent.
• Current valid Certificate of Practice from a relevant health-related College.
• Five years of practical and related clinical experience.
• Demonstrated knowledge and experience in discharge planning and case management.
• Proficiency in utilization management principles and length of stay management strategies.
• Excellent organizational, analytical, and communication skills.

Preferred Qualifications

• Experience in leadership and project management.
• Experience in program development and implementation.
