Director of Clinical Services (RN)

Responsibilities

• Own clinical execution for the branch, monitoring patient status and ensuring timely follow-up
• Coordinate next steps after clinical events such as new symptoms, hospitalizations, or discharges
• Ensure care plans are clear, actionable, and compliant for field clinicians
• Coach and mentor field clinicians on clinical reasoning, visit planning, and documentation quality
• Set and reinforce expectations for reliability, professionalism, and patient experience
• Identify recurring clinical issues and implement process improvements
• Serve as a decisive escalation point for clinical questions and patient risk

Requirements

• Minimum of one year of recent clinical experience as a Registered Nurse
• At least one year of experience in a Home Health setting
• Ability to coach, mentor, and train clinical staff
• Strong clinical judgment and structured approach to prioritization
• Proficiency with Homecare Homebase (EHR)
• Understanding of Medicare Conditions of Participation (CoPs) and OASIS documentation

Preferred Qualifications

• Previous leadership or supervisory experience in home health
• Experience building or improving clinical workflows at scale
• Experience partnering with operations teams to prevent service failures
• Experience with utilization review and quality assurance
