About the Company

Durham Community Health Centre (DCHC) is a registered charitable organization that provides integrated, accessible, and equitable community-based primary care, wellness services, and health education to community members in Durham who face barriers to health and well-being.

Responsibilities

• Provide strategic and operational leadership for designated Hub models and community-based programs
• Lead the strategic development, implementation, and sustainability of models of care
• Provide executive oversight of services to ensure safe, accessible, and equitable care
• Lead people leadership, coaching, and mentorship for managers and interdisciplinary teams
• Develop and maintain meaningful relationships with community groups, Indigenous partners, and stakeholders
• Partner with corporate service teams to support integrated planning and resource stewardship
• Monitor key performance indicators and use evidence to drive continuous improvement
• Manage assigned operational budgets and ensure financial sustainability

Requirements

• Undergraduate degree in health administration, health sciences, nursing, social work, public health, or related discipline
• 5-7 years of progressive leadership experience in health care, community health, or social services
• Demonstrated success leading interdisciplinary teams and complex integrated programs
• Strong understanding of health equity, trauma-informed care, and social determinants of health
• Knowledge of Ontario's health system and community health centre frameworks
• Proven ability to build collaborative relationships with diverse stakeholder groups
• Valid Ontario driver's licence and access to a reliable vehicle
• Full vaccination against COVID-19

Preferred Qualifications

• Master's degree in a relevant field
• Experience with HART Hubs or Youth Wellness Hubs Ontario
• Experience leading outreach services or strategic community integration initiatives
