About the Company

Durham Community Health Centre (DCHC) is a registered charitable organization that provides integrated, accessible, and equitable community-based primary care, wellness services, and health education to community members in Durham who face barriers to health and well-being.

Responsibilities

  • Provide strategic and operational leadership for primary care, clinical services, and integrated care programs
  • Lead the development and implementation of integrated care models and pathways
  • Provide executive oversight of clinical and community-based services to ensure high-quality care
  • Offer leadership, coaching, and mentorship to managers and interdisciplinary teams
  • Champion person-centred models of care that embed health equity and trauma-informed practice
  • Develop and sustain strategic partnerships with health service providers, hospitals, and community agencies
  • Monitor key performance indicators and use evidence to drive continuous quality improvement
  • Manage assigned operational budgets and ensure financial stewardship

Requirements

  • Undergraduate degree in health administration, health sciences, nursing, social work, public health, or business administration
  • 5-7 years of progressive leadership experience in healthcare or related sectors
  • Demonstrated success leading interdisciplinary teams and complex integrated health services
  • Strong understanding of health equity, trauma-informed care, and social determinants of health
  • Knowledge of Ontario's health system and community health centre frameworks
  • Experience in strategic planning, budget oversight, and risk management
  • Valid Ontario driver's licence and access to a reliable vehicle
  • Full vaccination against COVID-19

Preferred Qualifications

  • Master's degree in a relevant field
  • Experience leading primary care or clinical services specifically
