Diabetes Educator

Responsibilities

• Support patients with type 2 diabetes through virtual and in-person care
• Develop and implement care plans across diet, exercise, sleep, and stress management
• Collaborate with prescribing providers on medication choices
• Use motivational interviewing and CGM data to help patients improve outcomes
• Coordinate care with behavioral health, exercise, and health coaching teams
• Counsel patients and families via video, phone, in person, and messaging

Requirements

• Registered Dietitian (RD) or Registered Nurse (RN)
• Certified Diabetes Care and Education Specialist (CDCES)
• Experience using CGM data to support blood sugar control
• Strong interpersonal and patient-coaching skills
• Ability to manage multiple priorities and changing processes
• Commitment to team-based chronic disease care
