About the role
Responsibilities
- Support patients virtually and in person at current and future clinics
- Develop and implement care plans across diet, exercise, sleep, and stress management
- Collaborate with prescribing providers on medication choices
- Use motivational interviewing and CGM data to support patient goals
- Coordinate care with behavioral health, exercise specialists, and health coaches
- Counsel patients and families via video, phone, in-person, and messaging
Requirements
- Registered Dietitian (RD) or Registered Nurse (RN)
- Certified Diabetes Care and Education Specialist (CDCES)
- Experience using CGM data to improve blood sugar control
- Strong interpersonal and patient-coaching skills
- Ability to manage multiple priorities and details
- Adaptable, self-motivated, team-oriented
