Customer Care Manager - Home Care

Responsibilities

• Conduct client assessments and develop personalised care plans to support independence
• Coordinate services that align with client goals and monitor service delivery outcomes
• Manage Home Care Package budgets, funding, and service agreements
• Build and maintain strong relationships with clients, families, and referral partners
• Perform regular care reviews and maintain accurate documentation
• Collaborate with Recruitment, Rostering, and Service Delivery teams to ensure exceptional care
• Support client onboarding and respond to customer enquiries or concerns
• Maintain compliance with aged care legislation and company policies

Requirements

• Experience coordinating services or managing a caseload
• Experience within the community services, aged care, or disability industry
• Strong relationship management and customer service skills
• Ability to work effectively with internal multidisciplinary teams

Preferred Qualifications

• Knowledge of Home Care Packages (HCP)
• Familiarity with the Support at Home program
