About the role
Responsibilities
- Facilitate intensive wrap-around services and care coordination for youth and families within the WISe Program
- Develop and implement goals and objectives based on CANS assessments and Child and Family Team (CFT) meetings
- Implement Evidence Based Practices to meet the individual needs of youth and families using a strength-based recovery model
- Coordinate and guide CFT treatment teams and communicate regularly with all involved service providers
- Conduct community outreach to educate referral sources about the program
- Collaborate with community organizations and schools to ensure a coordinated family support program
- Advocate for the values of a diverse population and ensure services are culturally and linguistically compatible with participants
Requirements
- Bachelor's degree in education, psychology, social work, or a related field
- Three years of community or educational outreach experience
- Two years of direct service experience working with children and families
- Active Washington State Registered Agency Affiliated Counselor Credential
- Valid Washington State Driver’s License and a safe, reliable vehicle with proof of insurance
- CPR and First Aid Certifications
- Ability to pass a background check and provide an acceptable driving abstract
Preferred Qualifications
- Knowledge of other state and federal support programs
- Understanding of medical and clinical terminology
- Strong attention to detail and accuracy
- Knowledge of County and Agency policies and procedures
