Coordinator - WISe Program

Responsibilities

• Facilitate intensive wrap-around services and care coordination for youth and families within the WISe Program
• Develop and implement goals and objectives based on CANS assessments and Child and Family Team (CFT) meetings
• Implement Evidence Based Practices to meet the individual needs of youth and families using a strength-based recovery model
• Coordinate and guide CFT treatment teams and communicate regularly with all involved service providers
• Conduct community outreach to educate referral sources about the program
• Collaborate with community organizations and schools to ensure a coordinated family support program
• Advocate for the values of a diverse population and ensure services are culturally and linguistically compatible with participants

Requirements

• Bachelor's degree in education, psychology, social work, or a related field
• Three years of community or educational outreach experience
• Two years of direct service experience working with children and families
• Active Washington State Registered Agency Affiliated Counselor Credential
• Valid Washington State Driver’s License and a safe, reliable vehicle with proof of insurance
• CPR and First Aid Certifications
• Ability to pass a background check and provide an acceptable driving abstract

Preferred Qualifications

• Knowledge of other state and federal support programs
• Understanding of medical and clinical terminology
• Strong attention to detail and accuracy
• Knowledge of County and Agency policies and procedures
