About the role
Responsibilities
- Provide ongoing assessment of consumer needs and facilitate service plan updates every 180 days
- Support consumers in crisis situations and assist with benefit applications (SSI, Medicaid, Section 8)
- Help consumers locate community resources, affordable housing, and public transportation
- Accompany consumers to medical and psychiatric appointments
- Monitor medication compliance and symptom self-management
- Document all services and progress notes in the Electronic Medical Record (EMR)
- Participate in the on-call system for after-hours, holiday, and weekend support
- Advocate for consumers navigating social and governmental systems
Requirements
- High school diploma required
- 1 to 4 years of experience in social services or a nonprofit environment
- Experience in a mental health agency is highly desired
- Ability to work flexible hours including evenings and weekends
- Proficiency with Microsoft Office Suite and Electronic Medical Record (EMR) systems
- Strong organizational and multitasking skills
Preferred Qualifications
- Knowledge of ethical practices related to mental health care
- Ability to relate to adults with serious and persistent mental illness
