About the role

Responsibilities

  • Provide ongoing assessment of consumer needs and facilitate service plan updates every 180 days
  • Support consumers in crisis situations and assist with benefit applications (SSI, Medicaid, Section 8)
  • Help consumers locate community resources, affordable housing, and public transportation
  • Accompany consumers to medical and psychiatric appointments
  • Monitor medication compliance and symptom self-management
  • Document all services and progress notes in the Electronic Medical Record (EMR)
  • Participate in the on-call system for after-hours, holiday, and weekend support
  • Advocate for consumers navigating social and governmental systems

Requirements

  • High school diploma required
  • 1 to 4 years of experience in social services or a nonprofit environment
  • Experience in a mental health agency is highly desired
  • Ability to work flexible hours including evenings and weekends
  • Proficiency with Microsoft Office Suite and Electronic Medical Record (EMR) systems
  • Strong organizational and multitasking skills

Preferred Qualifications

  • Knowledge of ethical practices related to mental health care
  • Ability to relate to adults with serious and persistent mental illness
