Community Services Coordinator

Responsibilities

• Facilitate the care process for at-risk and medically complex patients by addressing social determinants of health (SDoH).
• Coordinate accessible plans of care by connecting patients and families to specialty care, community resources, and social service agencies.
• Screen patients for clinical and social triggers such as transportation, housing, nutrition, and disability support.
• Conduct high-volume patient outreach via telephone, patient portal, and in-person or virtual encounters.
• Document all outreach, care coordination activities, and patient information accurately in the electronic medical record (EMR).
• Collaborate with primary care teams, case managers, social workers, and community partners to ensure continuity of care.
• Track patient adherence to appointments, laboratory tests, referrals, and prescription compliance.

Requirements

• 1-3+ years of experience in a clinical setting.
• High School diploma or equivalency certificate (e.g., GED, HiSET, TASC).
• Active Driver's license and reliable transportation.

Preferred Qualifications

• Previous experience working in healthcare or human services.
• Experience using Electronic Medical Record (EMR) systems.
• Bachelor's degree in a related field such as Healthcare Administration, Psychology, Sociology, or Human Services.
• Bi-lingual capabilities.
