About the role

Responsibilities

  • Provide psychosocial support to homeless youth aged 17 to 22 in a residential program
  • Support consumers to develop individual support plans in partnership with mental health services and carers
  • Liaise and collaborate with community support agencies, including housing and mental health services
  • Monitor and review consumer progress against case plans and provide regular progress reports
  • Observe and accurately record client behavior, needs, and interactions
  • Maintain accurate case notes, up-to-date case plans, and client files
  • Coordinate the delivery of services and assist clients in utilizing referral agencies

Requirements

  • Certificate IV in Mental Health
  • Experience working with individuals experiencing mental health issues and homelessness
  • Demonstrated ability to work with people from diverse cultural backgrounds, including Aboriginal and Torres Strait Islander people
  • Ability to engage with people with complex needs and psychiatric disabilities
  • Current First Aid Certificate
  • Current Driver Licence
  • Ability to work a 24/7 roster, including days, afternoons, and occasional active night shifts
