Responsibilities

• Provide essential support for patient discharges within the Integrated Transfer of Care Team
• Conduct thorough community-based assessments
• Ensure appropriate provision of equipment for patients
• Travel within the community to visit patients to ensure smooth transitions from hospital to home
• Work closely with multidisciplinary healthcare professionals to deliver comprehensive care

Requirements

• Experience in community physiotherapy
• Valid driver’s license and ability to travel within the community
• Relevant professional qualifications and registration
• Strong communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
