Community Physiotherapist

Responsibilities

• Conduct comprehensive assessments of patients in the community to identify rehabilitation needs
• Develop and implement individualized, evidence-based treatment plans
• Evaluate interventions to improve patient mobility, function, and quality of life
• Collaborate with multidisciplinary teams to ensure coordinated patient care
• Monitor and document patient progress and adjust treatment plans as necessary
• Provide education and support to patients and families regarding self-management strategies

Requirements

• HCPC registration
• Experience in a healthcare, therapy, or rehabilitation environment
• Strong communication and interpersonal skills
• Ability to work effectively within a multidisciplinary team
• Compassionate and patient-centered approach
• Flexibility and adaptability
