Community Physiotherapist

Responsibilities

• Provide comprehensive physiotherapy services within the Jean Hayes Reablement Unit
• Conduct thorough assessments of patients and develop personalised rehabilitation plans
• Deliver interventions to support recovery and improve functional independence
• Manage a caseload primarily consisting of frail and elderly patients
• Work collaboratively with other healthcare professionals to deliver integrated care
• Maintain accurate patient records and contribute to service development

Requirements

• Diploma or Degree in Physiotherapy
• HCPC registration as a Physiotherapist
• Right to work in the UK
• Experience in intermediate care and rehabilitation
• Proven ability to manage frail and elderly patients
• Experience in a community setting
• Strong clinical assessment and treatment planning skills
