Community Liaison - Home Health & Hospice

Responsibilities

• Develop and strengthen relationships with physicians, hospitals, skilled nursing facilities, and assisted living communities
• Conduct daily in-person outreach and educational visits throughout the assigned territory
• Identify new business opportunities and cultivate prospective referral sources
• Represent Granite VNA at networking events, community programs, and professional meetings
• Manage a portfolio of referral accounts and develop growth and retention strategies
• Monitor referral trends and collaborate with clinical and intake teams to support seamless transitions of care

Requirements

• Valid driver's license and reliable transportation for frequent local travel
• Proficiency with CRM systems and Microsoft Office applications
• Strong interpersonal, communication, and presentation skills
• Ability to work independently while managing multiple priorities

Preferred Qualifications

• Bachelor's degree in marketing, business, social work, or a related field
• 2+ years of healthcare sales, hospice, or home health business development experience
• Experience managing provider relationships and referral accounts
• Knowledge of home health, hospice, and healthcare referral processes
