About the role
Responsibilities
- Provide case management, system navigation, and advocacy to seniors returning to the community from federal correctional institutions.
- Conduct in-reach at federal institutions to identify program participants and participate in release planning.
- Develop and implement comprehensive case management plans addressing mental health, housing, addictions, and employment.
- Build and maintain professional relationships with Correctional Services Canada (CSC) officers, community agencies, and police.
- Recruit, onboard, and manage volunteers to support client success.
- Monitor client access to health services and ensure compliance with health regimens.
- De-escalate serious incidents and crises in a safe, holistic manner.
- Deliver program presentations to community agencies and stakeholders.
Requirements
- Post-Secondary Degree or Diploma in Correctional or Social Services.
- At least 2 years of relevant experience working in a Criminal Justice System (CJS) related environment.
- Proven experience in case management and using case management software.
- Valid Unrestricted Class G Driver’s License and access to a reliable vehicle.
- Ability to obtain and maintain Federal Security (Reliability) Clearance.
- Up-to-date First Aid/CPR certification.
- Non-Violent Crisis Prevention & Intervention Certificate (or willingness to undergo training).
- Strong understanding of professional boundaries when working with vulnerable populations.
Preferred Qualifications
- Knowledge of the Kingston community and local social service agencies.
- Proficiency in French language skills.
