About the role
Responsibilities
- Attend a virtual 2-week paid training program
- Meet patients in the community, home, and healthcare facilities to conduct needs assessments and motivate health goals
- Support patients with social challenges including housing instability, behavioral health concerns, and food insecurity
- Advocate for healthcare needs and assist with navigating medical services and resources
- Build and maintain a directory of community resources
- Partner with Waymark care delivery team and healthcare facility providers
- Document patient encounters accurately

Preferred Qualifications
- Community health worker certification
- Long-time resident of Norfolk/Portsmouth with local resource knowledge
- Home visit and outreach experience
- Managed care patient experience
- Knowledge of local Medicaid populations
