About the role

Responsibilities

  • Attend a virtual 2-week paid training program
  • Meet patients in the community, home, and healthcare facilities to conduct needs assessments and motivate health goals
  • Support patients with social challenges including housing instability, behavioral health concerns, and food insecurity
  • Advocate for healthcare needs and assist with navigating medical services and resources
  • Build and maintain a directory of community resources
  • Partner with Waymark care delivery team and healthcare facility providers
  • Document patient encounters accurately
