Community Health Worker

Responsibilities

• Attend a virtual 2-week paid training program
• Meet patients in the community, home, and healthcare facilities to conduct needs assessments and motivate health goals
• Support patients with social challenges including housing instability, behavioral health concerns, and food insecurity
• Advocate for healthcare needs and assist with navigating medical services and resources
• Build and maintain a directory of community resources
• Partner with Waymark care delivery team and healthcare facility providers
• Document patient encounters accurately

Requirements

• Highly organized and self-motivated
• Sound judgment and quick situational analysis
• Empathetic, passionate, and professional demeanor
• Excellent communication skills
• Available for weekend and late-start shifts
• Reside within commutable distance of Norfolk/Portsmouth, Virginia
• Travel up to 80% within surrounding counties
• Current driver's license and access to an insured vehicle

Preferred Qualifications

• Community health worker certification
• Long-time resident of Norfolk/Portsmouth with local resource knowledge
• Home visit and outreach experience
• Managed care patient experience
• Knowledge of local Medicaid populations
