About the role

Responsibilities

  • Meet patients in the community, in the home, and in healthcare facilities to conduct needs assessments
  • Help patients address health-related social issues like homelessness, substance use, and hunger
  • Create and maintain a directory of community resources such as food banks and housing assistance
  • Manage relationships with healthcare providers and partner with multidisciplinary care teams
  • Accompany members to medical appointments and navigate technology systems for documentation
  • Support outreach to eligible individuals via in-person, phone, or video visits
  • Participate in weekly care team huddles

Requirements

  • Reside within a commutable distance of Cleveland
  • Current Driver’s license and access to an insured vehicle
  • Ability to travel within surrounding counties (up to 80%)
  • Availability for a set schedule including at least one weekend day and one late-start shift per week
  • Computer literacy for virtual communication and documentation
  • Strong interpersonal communication and active listening skills
  • Cultural competency to work with diverse community groups

Preferred Qualifications

  • Community Health Worker certification
  • Long-time resident of the Greater Cleveland area
  • Experience conducting home visits and outreach
  • Experience working with managed care patients
  • Knowledge of Greater Cleveland Medicaid populations
