Sabathani Community Center (SCC) is one of Minnesota’s oldest African American-founded nonprofits & serves a multi-cultural community of more than 50,000 community members in South Minneapolis. SCC was founded in 1966 to address social and economic disparities in underserved communities in the areas of youth programming, housing, health and education.

Responsibilities

• Administer PRAPARE, 4Ps, and EPDS screenings for newly enrolled participants
• Develop individualized care plans within 30 days of enrollment
• Maintain a caseload of approximately 15 participants through pregnancy and 12 months postpartum
• Conduct weekly participant check-ins via phone, home visit, or in person
• Facilitate warm referrals to health, behavioral health, housing, and social service providers
• Coordinate with doulas and mental health providers to ensure integrated support
• Assess and support basic needs including transportation, housing, and infant provisions
• Deliver 1:1 education on nutrition, breastfeeding, and postpartum wellness
• Document all encounters and data in Salesforce within 48 hours

Requirements

• High school diploma or equivalent
• Community Health Worker (CHW) certification
• Minimum 2 years of experience in community health, case management, or doula work
• Valid MN Driver's License and access to reliable transportation
• Ability to pass a criminal background check

Preferred Qualifications

• Associate's or Bachelor's degree in a related field
• Experience working with pregnant and postpartum Black/African American community members
• Experience with Salesforce or similar case management systems
• Lived experience and cultural connection to the community served
