Community Health Worker-Bilingual

Responsibilities

• Serve as a liaison between health/social services and the community to facilitate access to resources and improve health outcomes.
• Provide health system navigation, including helping patients find providers, making appointments, and arranging transportation.
• Conduct outreach through home visits, hospital visits, and community engagement to meet patients where they are.
• Deliver health promotion and coaching on topics such as tobacco cessation, nutrition, physical fitness, and chronic disease management.
• Provide culturally appropriate health education to empower clients and increase community self-sufficiency.
• Coordinate with medical providers and primary care teams to provide care coordination and reduce emergency room visits.
• Assist patients with obtaining necessary screenings and prerequisites for medical services.

Requirements

• Associate's degree or higher.
• Completed an approved South Carolina CHW training program or equivalent.
• Valid driver's license and reliable personal transportation.
• Proficiency in MS Office Suite (Outlook, Word, Excel, etc.).
• Basic knowledge of medical terminology.
• Current BLS certification.
• Ability to pass a thorough background investigation.
• Bilingual skills (as specified in job title).

Preferred Qualifications

• Minimum of two years of direct experience in community health.
• Experience navigating local medical and social support systems.
• Experience using Electronic Medical Records (EMR) software.
