About the role
Responsibilities
- Deliver public health and case management services using initiative, judgment, and independent decision-making within professional nursing standards
- Manage admissions and visits for the Rural Health Clinic Nurse program
- Execute school nurse assignments as directed by the PHN Community Health Manager
- Complete grant work plan activities in accordance with specific grant requirements
- Adhere to established policies and procedures when performing all assigned activities
- Maintain accurate charting according to grant and community health guidelines
Requirements
- Graduate of an accredited school of professional nursing
- Active Minnesota Registered Nurse (RN) license
- 1–3 years of experience in public health and/or home care
- Public Health Certification
- Basic Life Support (BLS) / CPR certification
- Experience with charting and case management
