About the role

Responsibilities

  • Bridge the gap between marginalized communities (specifically Black, Latinx, and LGBTQIA2S+ populations) and the healthcare system through outreach and education.
  • Plan and execute outreach events, workshops, and activities at various community venues to promote health equity and awareness.
  • Establish and maintain relationships with community leaders, organizations, and local businesses to enhance access to sexual and reproductive health services.
  • Maintain the LGBTQ+ affirming referral directory and assist clients with internal and external referrals for behavioral health, primary care, and other human services.
  • Develop and deliver training on LGBTQ+ healthcare access for both external agencies and internal staff.
  • Support program administration, including data collection, reporting, and maintaining documentation in Salesforce and other platforms.
  • Travel across seven counties (Fulton, Montgomery, Saratoga, Schenectady, Schoharie, Warren, and Washington) to conduct regional outreach.

Requirements

  • Bachelor's degree in social work, psychology, counseling, or a related field preferred.
  • Two years of relevant experience in navigation or referral services if no college degree is held.
  • Strong knowledge of the barriers faced by LGBTQ+ individuals in accessing healthcare and human services.
  • Proficiency in Microsoft Excel, Notion, and Word.
  • Experience working with electronic health records (EHR).
  • Ability to travel extensively across the assigned seven-county region.
  • Demonstrated commitment to racial equity, gender justice, and bodily autonomy.

Preferred Qualifications

  • Coursework specifically focused on LGBTQ+ issues.
  • Experience with Salesforce or similar database reporting systems.
