Community Health Nurse

Responsibilities

• Partner with primary care clinics and community organizations to lead quality improvement initiatives focused on hypertension management.
• Deliver patient education in community and clinical settings to promote self-management and healthy lifestyle changes.
• Collect, analyze, and report clinical and quality data to drive continuous improvement efforts.
• Act as a clinical liaison among patients, providers, and community stakeholders to improve care coordination.
• Conduct outreach activities to increase program participation and awareness in target Chicago neighborhoods.
• Support Community Health Worker (CHW) efforts through education and participant triage.
• Collaborate with interdisciplinary teams to support program planning and sustainability.

Requirements

• Current, unrestricted Registered Nurse (RN) license in Illinois.
• 3-5 years of nursing experience in community health, public health, or ambulatory care.
• Experience with chronic disease management, particularly hypertension.
• Proficiency with electronic health records (EHR) and data management systems.
• Proven ability to work independently with strong clinical judgment and organizational skills.
• Cultural competency and experience serving diverse, underserved populations.
• Willingness to travel within Chicago for community outreach and clinic support.

Preferred Qualifications

• Certification in Public Health Nursing or a related specialty.
• Direct experience working in community-based health programs.
