About the role

Responsibilities

  • Deliver culturally responsive community health education modules addressing chronic disease, nutrition, mental health, and maternal/child health.
  • Build and sustain relationships with residents, community leaders, and grassroots organizations in Macon, Barbour, and Bullock Counties.
  • Conduct community needs assessments to identify health priorities and barriers to care.
  • Organize and facilitate community health forums, listening sessions, and health events.
  • Collaborate with university faculty, Cooperative Extension, local health departments, and HBCU partners.
  • Support youth and family health initiatives through 4-H and other Extension programs.
  • Train and support community health workers, volunteers, and local organizations.
  • Assist with grant writing, partnership development, and resource acquisition.
  • Monitor and evaluate program outcomes using participatory and culturally appropriate methods.
  • Prepare impact reports, success stories, and presentations for stakeholders.

Requirements

  • Bachelor's degree in Public Health, Health Education, Social Work, Community Development, or a related field (Master's degree preferred for Level II; Master's required for Level III).
  • Experience in community health education, outreach, or related work (Years of experience required based on level: 2-4 years for Level I, 5-7 years for Level II, or 8-10+ years for Level III).
  • Knowledge of community health principles, health equity, and social determinants of health.
  • Ability to travel locally within a multi-county service area.
  • Ability to work evenings and weekends as required by community needs.

Preferred Qualifications

  • Master of Science degree in Public Health or a closely related field.
  • Experience with Cooperative Extension or land-grant mission models.
  • Experience in program design, evaluation, and partnership development.
