About the Company

Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Their clinicians provide a continuum of care for patients with chronic conditions and limited mobility directly in their homes.

Responsibilities

• Provide oversight of physician services and medical components of care
• Conduct annual wellness exams for patients
• Act as a medical resource to the interdisciplinary group (IDG)
• Ensure continuity of hospice medical services and appropriate symptom control
• Certify and re-certify patients for hospice admission and Medicare Hospice Benefit
• Document care provided in clinical records, providing evidence of disease progression
• Collaborate with the IDG to ensure medical needs are met
• Review and develop protocols for treatment and interventions
• Serve as a liaison to physicians in the community

Requirements

• Licensed or authorized to work in South Carolina
• MD or DO degree licensed to practice medicine
• Certified in Hospice or a related specialty
• Experience with end-of-life care
• Maintains controlled substances registration with state and federal authorities
• Not excluded from the Medicare program
• Strong verbal and written communication skills
