About the role
About the Company
Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Their clinicians provide a continuum of care for patients with chronic conditions and limited mobility directly in their homes.
Responsibilities
- Provide oversight of physician services and medical components of care
- Conduct annual wellness exams for patients
- Act as a medical resource to the interdisciplinary group (IDG)
- Ensure continuity of hospice medical services and appropriate symptom control
- Certify and re-certify patients for hospice admission and Medicare Hospice Benefit
- Document care provided in clinical records, providing evidence of disease progression
- Collaborate with the IDG to ensure medical needs are met
- Review and develop protocols for treatment and interventions
- Serve as a liaison to physicians in the community
Requirements
- Licensed or authorized to work in South Carolina
- MD or DO degree licensed to practice medicine
- Certified in Hospice or a related specialty
- Experience with end-of-life care
- Maintains controlled substances registration with state and federal authorities
- Not excluded from the Medicare program
- Strong verbal and written communication skills
