Collaborating Physician

Responsibilities

• Provide oversight of physician services and medical components of care
• Conduct annual wellness exams for patients
• Act as a medical resource to the interdisciplinary group (IDG)
• Ensure continuity of hospice medical services and appropriate symptom control
• Certify and re-certify patients for hospice admission and Medicare benefits
• Document care provided in clinical records, including disease progression
• Collaborate with the IDG to ensure medical needs are met
• Participate in performance improvement programs and clinical/ethical issue resolution
• Act as a liaison to community physicians

Requirements

• MD or DO degree
• Licensed or authorized to work in NY, NJ, MD, VA, GA, FL, OK, KS, TX, or PA
• Certified in Hospice or a related specialty
• Experience with end-of-life care
• Valid controlled substances registration with state and federal authorities
• Not excluded from the Medicare program
• Strong verbal and written communication skills
