Responsibilities

• Provide direct patient care and assist with activities of daily living (ADLs) including bathing, grooming, oral hygiene, and skin care.
• Monitor and record vital signs such as blood pressure, pulse, respiration, weight, and temperature, reporting abnormalities to the RN.
• Assist patients with positioning, transfers, ambulation, and the use of medical equipment like Hoyer lifts, commodes, and wheelchairs.
• Maintain a clean, safe, and healthy environment for patients to promote optimal well-being.
• Document patient health status and services provided accurately and descriptively.
• Report changes in patient condition, accidents, or grievances immediately to the Case Manager or Clinical Manager.
• Offer emotional support to patients and families in accordance with hospice philosophy.

Requirements

• Current Florida CNA certification.
• Current CPR certification.
• Valid Florida Driver’s License and valid auto insurance.
• Experience caring for terminally ill patients or those with life-threatening illnesses.
• Ability to work a part-time schedule (shift is 2 days per week).

Preferred Qualifications

• Minimum of one (1) year of professional experience in a clinical setting.
