About the role

Responsibilities

  • Provide direct patient care and assist with activities of daily living (ADLs) in homes, facilities, or inpatient units.
  • Perform personal care tasks including bathing, skin care, dressing, nail care, and oral hygiene.
  • Monitor and record vital signs such as blood pressure, pulse, respiration, weight, and temperature.
  • Assist patients with mobility, positioning, transfers, and the use of equipment like Hoyer lifts or wheelchairs.
  • Maintain a clean, safe, and healthy environment for patients to promote optimal well-being.
  • Document patient health status and services provided accurately and descriptively.
  • Report changes in patient condition, accidents, or grievances immediately to the Clinical Care Manager or RN.
  • Offer emotional support to patients and families in accordance with hospice philosophy.

Requirements

  • Current Florida CNA certification.
  • Current CPR certification.
  • Experience with terminally ill patients or those with life-threatening illnesses.
  • Valid Florida Driver’s License.
  • Valid Auto Insurance.

Preferred Qualifications

  • Minimum of one (1) year of experience in a clinical or home health setting.
