CMO - Discharge Coordinator

Responsibilities

• Coordinate and manage patient discharges to ensure safe, timely, and efficient transitions of care
• Support case management activities by assessing patient needs and planning appropriate discharge pathways
• Monitor patient flow and collaborate with clinical teams to maintain continuity of care
• Communicate effectively with multidisciplinary teams to align discharge plans and resolve barriers
• Liaise with patients, families, insurance providers, and external healthcare facilities to facilitate smooth transitions
• Maintain accurate documentation in EMR systems and ensure compliance with regulatory and quality standards
• Prioritize and manage multiple discharge cases simultaneously in a fast-paced environment

Requirements

• Licensed Nurse with a valid professional nursing license or a relevant medical background
• 2–3 years of experience in discharge planning, case management, care coordination, or a similar healthcare role
• Proficiency in EMR systems and Microsoft Office
• Strong understanding of healthcare regulations and documentation standards
• Strong communication, negotiation, and interpersonal skills

Preferred Qualifications

• Arabic language proficiency for enhanced patient engagement and culturally sensitive support
