About the role

Responsibilities

  • Coordinate care for patients in skilled nursing and rehabilitation facilities using EMR
  • Facilitate transitional care management to ensure immediate service initiation upon hospital discharge
  • Monitor patients for changes in condition and escalate care as clinically indicated
  • Collaborate with providers and facility staff to maintain current care plans
  • Conduct routine and urgent vital sign collection and patient check-ins
  • Serve as a liaison between providers, facility staff, residents, and families
  • Accurately document patient interactions and perform chart reconciliation
  • Support onboarding and act as a clinical mentor for new coordinators

Requirements

  • Active, unrestricted CMA license in the state of Texas
  • Minimum 1 year of patient-facing experience in long-term care, skilled nursing, or transitional care
  • Proficiency with Microsoft 365 (Excel, Outlook, Word, Teams)
  • Experience with EMR systems (Point Click Care preferred)
  • Valid driver's license and reliable personally-owned vehicle with insurance
  • Ability to work on-site 5 days per week in the Temple, Texas area
  • Ability to pass background check and drug screen
