Clinical Services Director

Responsibilities

• Manage the Habitat Health clinic, rehabilitation services, social work, and behavioral health services
• Provide ongoing clinical guidance and support to the Interdisciplinary Team (IDT)
• Direct clinic and social work operations including hiring, training, supervising, and mentoring team members
• Ensure smooth care transitions between hospitals, skilled nursing facilities, and home settings
• Maintain compliance with PACE, ADHC, and DHCS regulations
• Lead the clinical quality improvement process and achieve key performance indicators
• Fill gaps in direct participant care as needed

Requirements

• Active and unrestricted Registered Nurse (RN) License in California
• Minimum 8 years of experience in healthcare delivery or operations
• 5+ years of experience in a supervisory or administrative capacity
• Experience in complex brick and mortar healthcare delivery
• Current CPR/BLS certification (or ability to obtain within 30 days)
• Valid CA driver’s license, personal transportation, and good driving record
• Proof of medical clearance for communicable diseases and up-to-date immunizations

Preferred Qualifications

• Bachelor's Degree in nursing, healthcare administration, or a related field
• Bilingual in Spanish
• Experience leading multidisciplinary healthcare teams
• Experience with PACE, ADHC, or value-based care models
• Experience in or leading case management functions
