About the role
Responsibilities
- Provide comprehensive medication management including reconciliation, dose optimization, and adherence support
- Partner with physicians and interdisciplinary teams to order individualized care plans under a CPA
- Monitor therapy effectiveness and safety to identify optimization opportunities
- Support quality-improvement and REMS initiatives
- Assist operations with complex prior authorizations and workflow improvements
Requirements
- Doctor of Pharmacy (PharmD)
- Licensed pharmacist in Oregon
- Completion of a PGY2 Ambulatory Care Residency, OR completion of a PGY1 Residency with 1 year of ambulatory practice under a CPA, OR at least 3 years of ambulatory practice with disease state management
- Proficiency in Microsoft Office tools
