About the role
Responsibilities
- Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments
- Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA
- Monitor therapy effectiveness and safety, identify optimization and intervene as appropriate
- Support quality-improvement and REMS initiatives
- Provide recommendations to operations by supporting teammates with complex prior authorizations and improving workflows
Requirements
- Doctor of Pharmacy (PharmD)
- Licensed pharmacist in Washington
- Completion of a PGY2 Ambulatory Care Residency, OR completion of a PGY1 Residency and 1 year of ambulatory practice under a CPA, OR at least 3 years of ambulatory practice with disease state management
- Proficiency in Microsoft Office tools (Outlook, Teams, Word, Excel, and PowerPoint)
