Clinical Pharmacy Specialist

Responsibilities

• Provide expert drug therapy design, monitoring, and patient medication counseling
• Participate in Collaborative Drug Therapy Management (CDTM) within inpatient and outpatient settings
• Serve as a therapeutic resource for drug information and pharmacotherapy for the Adult BMT service
• Participate in committees to develop treatment guidelines and pharmacy policies
• Provide preceptorship for PGY2 Pharmacy residents and pharmacy students
• Educate hospital staff through Grand Rounds presentations and in-service training

Requirements

• Doctorate in Pharmacy (PharmD)
• Current New York State pharmacy license or eligibility for licensure
• Completion of an ASHP-accredited pharmacy practice residency
• Completion of an ASHP-accredited oncology residency (or equivalent experience)

Preferred Qualifications

• Prior experience in oncology pharmacy practice
