Clinical Pharmacy Specialist (Hematology/Oncology)

Responsibilities

• Conduct comprehensive assessments for disease management under collaborative practice agreements or clinic protocols
• Work on-site in close coordination with interdisciplinary teams
• Assist in the development and maintenance of treatment plans and supportive care plans
• Monitor and evaluate medication regimens for therapeutic goals, adverse effects, and drug interactions
• Ensure clinical criteria compliance with FDA standards and national guidelines
• Identify, resolve, and prevent medication-related problems
• Provide direct, individualized counseling on medication use and disease state education
• Address barriers to adherence including cost, access, and health literacy

Requirements

• Doctor of Pharmacy (PharmD)
• Active Louisiana pharmacist license
• Oncology Pharmacy Specialty Certification (BCOP) required within 1 year of hire
• PGY1 + PGY2 residency in specialty area OR PGY1 + 1 year practice experience OR 3 years of specialization experience
• Proficiency in Microsoft Office tools

Preferred Qualifications

• Experience with EPIC & Beacon Oncology treatment plans
