Clinical Pharmacist

Responsibilities

• Provide pharmaceutical care to inpatients on the Liver service, including those with end-stage liver disease, acute liver injury, and pre/post-liver transplant patients.
• Participate in multidisciplinary rounds, medication reconciliation, TPN management, and therapeutic drug monitoring.
• Manage immunosuppression for solid organ transplant patients and assist in evaluating candidates for liver transplant listing.
• Support staff development for nurses, physicians, and pharmacy personnel.
• Coordinate and conduct quality initiative projects and develop collaborative treatment and research protocols.
• Fulfill academic duties including providing didactic lectures and precepting PharmD students and PGY1/PGY2 residents.
• Engage in scholarly activities, including presentations and publications.

Requirements

• PharmD degree.
• PGY1 residency experience.
• PGY2 residency in solid organ transplantation (or equivalent experience).
• Evidence of, or eligibility for, Illinois pharmacist licensure.

• Board certification.
• Strong clinical knowledge base and analytical problem-solving skills.
• Excellent verbal and written communication skills.
• Strong organizational, time management, and planning abilities.
