About the role
Responsibilities
- Provide safe, effective, and prompt medication therapy for patients requiring home infusion services.
- Interpret, evaluate, and accurately dispense drug orders, assessing appropriateness of drug selection, dose, and potential interactions.
- Identify, resolve, and prevent potential drug-related problems and recommend therapeutic alternatives.
- Supervise pharmacy technicians to ensure proper preparation of drug products and efficient workflow management.
- Coordinate clinical and dispensing workflows within the home infusion pharmacy.
- Participate in the orientation, education, and mentoring of new staff members.
- Maintain compliance with JCAHO, ASHP standards, and federal/state licensing acts.
Requirements
- Graduate of an ACPE accredited College of Pharmacy.
- Eligible for licensure as a pharmacist in the state of Illinois (must complete licensure within 60 days of employment).
- Completion of an ASHP accredited PGY-1 Pharmacy Residency or 3+ years of hospital staffing experience.
Preferred Qualifications
- Doctor of Pharmacy (PharmD) or other advanced degree (MS, MBA).
- Clinical pharmacy experience specifically in Home Infusion.
- Extensive clinical experience and development in lieu of formal residency training.
