About the role

Responsibilities

  • Provide pharmaceutical care through active participation in multidisciplinary rounds
  • Design, implement, and monitor patient-specific pharmacotherapy, including pharmacokinetic, nutrition, and anticoagulation management
  • Assist in the development, implementation, and monitoring of clinical pharmacy practices for solid organ transplant services
  • Lead drug use projects, cost reduction initiatives, and quality improvement activities
  • Develop staff educational and training programs and assist in the clinical training of residents and students
  • Provide drug information and education to patients and caregivers
  • Participate in hospital committees related to solid organ transplant care

Requirements

  • Doctor of Pharmacy (PharmD) degree or equivalent combination of education and experience
  • Completion of a specialty residency in solid organ transplant, a fellowship, or equivalent training
  • Experience in clinical pharmacy practice
  • Current Michigan Pharmacist licensure
  • Possession of a Michigan Controlled Substances license

Preferred Qualifications

  • Solid Organ Transplantation Pharmacy Specialty Certification (BCTXP)
  • Ability to perform manual dexterity movements including IV preparation and participating in codes
  • Fluency in English (reading, writing, and speaking)
