About the role
Responsibilities
- Provide pharmaceutical care through active participation in multidisciplinary rounds
- Design, implement, and monitor patient-specific pharmacotherapy, including pharmacokinetic, nutrition, and anticoagulation management
- Assist in the development, implementation, and monitoring of clinical pharmacy practices for solid organ transplant services
- Lead drug use projects, cost reduction initiatives, and quality improvement activities
- Develop staff educational and training programs and assist in the clinical training of residents and students
- Provide drug information and education to patients and caregivers
- Participate in hospital committees related to solid organ transplant care
Requirements
- Doctor of Pharmacy (PharmD) degree or equivalent combination of education and experience
- Completion of a specialty residency in solid organ transplant, a fellowship, or equivalent training
- Experience in clinical pharmacy practice
- Current Michigan Pharmacist licensure
- Possession of a Michigan Controlled Substances license
Preferred Qualifications
- Solid Organ Transplantation Pharmacy Specialty Certification (BCTXP)
- Ability to perform manual dexterity movements including IV preparation and participating in codes
- Fluency in English (reading, writing, and speaking)
