Clinical Pharmacist

Responsibilities

• Monitor the rational use of drugs and provide drug services to patients, families, and health workers
• Develop a drug formulary in collaboration with doctors and policy holders
• Conduct drug assessments, ward visits, and monitoring activities to ensure healthy drug use
• Design, implement, and monitor drug therapy to identify Drug Related Problems (DRP)
• Provide consultations to doctors regarding drug interactions, side effects, and dosage
• Provide education and training to pharmacists and other health workers
• Educate patients and families regarding medication and health information
• Collect data and create reports related to operational activities
• Implement quality and safety procedures to support hospital indicators

Requirements

• Master Degree in Clinical Pharmacy
• At least 1 year of experience as a Clinical Pharmacist in a hospital preferred
• In-depth knowledge of pharmacology and therapeutic drug monitoring
• Strong understanding of patient care principles
• Excellent communication skills for interacting with patients and healthcare professionals
• Strong analytical skills for evaluating medication therapy
