Responsibilities
- Perform clinical pharmacy services such as drug reconciliation
- Monitor drug therapy and provide patient counseling
- Conduct ward visits (visite)
- Actively participate in the pharmacy and therapy committee
- Participate in the antimicrobial resistance control committee
Requirements
- Master's degree (S2) in Clinical Pharmacy
- Active STR (Registration Certificate)
- Minimum 1 year of experience in a similar field
- Proficient in Microsoft Office (Word, Excel, PowerPoint)
- Strong sense of responsibility and communication skills
