Clinical Pharmacist

Responsibilities

• Monitor the rational use of drugs and provide drug services to patients, families, and health workers
• Develop hospital drug formularies and analyze new drugs in collaboration with doctors
• Conduct drug assessments, ward visits, and monitor medicine use to ensure healthy behavior
• Design and implement drug therapy while identifying potential Drug Related Problems (DRP)
• Provide consultations to doctors regarding drug interactions, side effects, and dosage
• Offer education and training to pharmacists, assistants, and other health workers
• Educate patients and families on drug use and health information
• Collect data and create operational reports for analysis
• Implement quality and safety procedures to support hospital-wide indicators

Requirements

• Master Degree in Clinical Pharmacy
• Minimum 1 year experience as a Clinical Pharmacist in a hospital preferred
• In-depth knowledge of pharmacology and therapeutic drug monitoring
• Strong understanding of patient care principles
• Excellent communication skills for interacting with patients and healthcare professionals
• Strong analytical skills for evaluating medication therapy
