Clinical Pharmacist

Responsibilities

• Provide day-to-day clinical pharmacy services across allocated wards, with a focus on critical care and/or cardiology
• Deliver medicines reconciliation, clinical screening and optimisation to improve patient outcomes
• Support safe and effective prescribing through clinical advice and therapeutic recommendations
• Participate in multidisciplinary ward rounds and liaise with medical and nursing staff
• Ensure compliance with local and national guidelines for medicines management
• Contribute to discharge planning, ensuring accurate and timely provision of medicines
• Promote antimicrobial stewardship and safe prescribing practices
• Support junior staff and participate in departmental learning and development activities

Requirements

• Registered Pharmacist with the GPhC
• Experience in critical care or cardiology
• Previous acute hospital clinical experience desirable
• Strong communication and multidisciplinary teamwork skills
• Ability to work independently and manage clinical priorities effectively
• Must hold the right to work in the UK
