About the role
Responsibilities
- Review prescriptions for clinical appropriateness, legality, and potential drug interactions
- Dispense and supply medications in line with Trust and legal standards
- Provide clinical and professional advice to medical, nursing, AHP, pharmacy staff, patients, and carers
- Make evidence-based clinical recommendations including dose adjustments and complex drug calculations
- Manage medication decisions for patients with renal/hepatic impairment or co-morbidities
- Advise in high-risk areas such as IV therapy, pregnancy, and breastfeeding
- Participate in pharmacy audits, research, and quality-improvement activities
- Support antimicrobial stewardship
- Implement Trust Medicines Management and medication safety policies
- Undertake duties of the Responsible Pharmacist when required
- Contribute to staff training and supervision of pharmacy students
Requirements
- MPharm or equivalent degree approved by the GPhC
- GPhC registered Pharmacist
- Postgraduate Diploma in Clinical Pharmacy or equivalent
- Minimum 1 year pre-registration training
- Strong clinical pharmacy and medicines management knowledge
- Computer literacy, ideally with Cerner and EMIS experience
- Experience in clinical audit and/or pharmacy practice research
- Eligibility to work in the UK
- Ability to work across multiple Trust sites
Preferred Qualifications
- Independent Prescriber qualification
